The influence of the target size on the sensitivity of the central visual field in quantitative static perimetry was studied using the program 32 of the automated perimeter Octopus 201. Seventy eyes of 70 normal subjects between 22 and 69 years of age were examined under the conditions of target sizes 1,3 and 5 which corresponded to the visual angles of 0.108°, 0.431° and 1.724°, respectively. Based on the average and standard deviation of sensitivity in each test point, the normal sensitivity of each test point for each age group in decade was standardized for target sizes 1,3 and 5. The influence of age on the profile of island of vision on 45° and 135° meridians was then studied in the same group of normal subjects. The normal sensitivity in each test point was found to decrease linearly for all target sizes with the peak sensitivity in the second and third decades. Mean sensitivity of 74 points of the Octopus program 32 decreased more with age for the target size 1 than for target sizes 3 and 5. As a clinical application of the above-mentioned results, quantitative static perimetry was performed on 32 eyes of 32 early glaucoma patients using the program 32 of Octopus 201 under the conditions of target sizes 1,3 and 5. In 10 out of 32 eyes, the target size 1 was more sensitive in detecting early glaucomatous visual field defects than target sizes 3 and 5. In conclusion the target size 1 in quantitative static perimetry was more sensitive in detecting early glaucomatous visual field defects and more useful in the diagnosis of early glaucoma than target sizes 3 and 5.
