Immunofluorescent and immunoelectron microscopic studies were carried out to determine the correlation between the progression of glomerular injury and expression of interstitial collagen, types I and III, in various types of glomerulonephritis. In addition, the localization of prolyl hydroxylase, the key enzyme of collagen synthesis, was examined to identifiy the collagen producing cells. In normal human kidney specimens, monoclonal antibodies against interstitial collagen (types I and III ) and prolyl hydroxylase did not react with any structure within the glomeruli. In various types of human glomerulonephritis, interstitial collagens were detected in the mesangial area, crescents and sclerosed glomeruli. The amounts and distribution of types I and III collagen in the mesangial area were associated with mesangial expansion. Type III collagen was stained in the moderately and severely expanded mesangium. Type I collagen was present only in the patients with severe mesangial expansion. In crescents and sclerosed glomeruli, type I collagen was colocalized with type III collagen. The staining intensity of type I collagen in these areas was generally stronger than that in the interstitium. Prolyl hydroxylase was colocalized with types I and/or III collagen by immunofluorescence. Immunogold staining revealed that prolyl hydroxylase was expressed in mesangial cells and glomerular epithelial cells. Observations on serially sliced specimens showed no evidence of migration of those collagens from interstitium through disrupted Bowman's capsule. These findings indicate that production of type III collagen is predominant in the early phase of glomerular injury and the ratio of type I to type III collagen is shifted toward a type I predominance during the progression of renal disease. Mesangial cells and glomerular epithelial cells play an important role in interstitial collagen synthesis in diseased glomeruli.
