The conditions for separate determination of trypsin-protein amidase (TPA) activity of α-1-antitrypsin (α 1AT) in the presence of α-2-macroglobulin were studied. Prior methylamine treatment of serum inactivated α-2-macroglobulin, but not α 1AT. TPA activities of α 1AT in sera of 85 patients with various cancers, 101 noncancerous patients and 42 healthy subjects were measured. The activities markedly increased in most cases of the advanced stage of gastric cancer as well as pancreatic and hepatic cancers. The activity was also significantly elevated in the early stage of gastric cancer (171.4±24.6U/ml n=18) as compared with the healthy subject (143.2±29.2U/ml n=41,P<0.001). The TPA activity was increased in 40% of noncancerous liver diseases. However, no significant relation was found, between the TPA activity and serum GOT, GPT or choline esterase.
