The median nerve somatosensory evoked potentials (SEPs) obtained by nerve stimulation were analyzed in 99 patients with various impairments of the central nervous system (CNS) which included 20 cervical lesions, 12 lesions of the posterior fossa, 14 lesions of the thalamus and/or basal ganglia, 16 hemispheric lesions, 23 head injuries, and 14 diffuse cerebral lesions in coma cases. Non-cephalic reference recording (NCR) and cephalic reference recording (CR) were conducted simultaneously to obtain evoked potentials. In the cases of cervical lesion, the P_<11>, or at times waves from P_<13> onward, disappeared in the presence of radiculopathy or myelopathy. For lesions in the medulla oblongata associated with impairments of the brainstem, waves from P_<13> onward disappeared. For lesions in the pons and the midbrain, waves from P_<14> onward disappeared. When the lesion in the vicinity of the midbrain was broad, waves from N_<16> onward disappeared. For disorders affecting the thalamus and the basal ganglia with lesions at either the postero-lateral region of the thalamus or posterior crus of internal capsule, the N_<16> was recorded while the P_<20> disappeared. By comparison of CRSEP and NCR-SEP, the location, range, and degree of seriousness of diseases could be estimated.
