Biliary bilirubin moieties in humans and rats were analyzed by using newly developed high performance liquid chromatography (HPLC). In healthy adults, seven peaks were obtained by HPLC. These seven peaks were studied by using thin layer chromatography and HPLC after diazotization and were identified as bilirubin diglucuronide (BDG), bilirubin monoglucuronide monoglucoside diester (BGG), bilirubin monoglucuronide monoxyloside diester (BGX), two peaks of bilirubin monoglucuronide (BMG), bilirubin IX_β and/or IX_δ, and bilirubin IX_α. The elution pattern was influenced by the pH of the eluate in the order of bilirubin IX_α, BMG and BDG. BDG accounted for 76.6% and BMG for 9.7% of the total bilirubin moieties in normal adults. In patients with Gilbert's syndrome and Crigler-Najjar syndrome type II, decreased BDG to 55.4%%, 12.9%, and increased BMG and bilirubin IX_α to 28.6%, 64.5% and to 1.1%, 11.0% were observed, respectively. The correlation between serum bilirubin level and BMG percentage in bile was good (r=0.916) in healthy adults and patients with unconjugated hyperbilirubinemia. In patients with Crigler-Najjar syndrome type II, marked decreases in BGG and BGX were also observed. The predominance of C-8 BMG to C-12 BMG was observed in constitutional hyperbilirubinemia when compared with the healthy adults, who excreted about equal amounts of C-8 and C-12 BMGs. In patients with cholelithiasis, bilirubin IX_α in gallbladder bile increased and deconjugation of bilirubin glucuronide was suggested. In a case of Gilbert's syndrome, BMG decreased and BDG increased in the bile after the treatment with phenobarbital (100 mg/day for 2 weeks), which suggests that induction of the hepatic bilirubin conjugating enzyme was caused by phenobarbital. In the analysis of rat bile, the biliary bilirubin composition of Wistar-Kyoto rat was similar to that of normal human adults. The Wistar rat and Sprague-Dawley rat showed low biliary BDG content, 54.1% and 49.4%, respectively. Gunn rats (heterozygote, Jj) excreted increased BMG 56.8%, and decreased BDG, 37.3%, in bile when compared with Wistar rats. It seemed to relate with the decrease in bilirubin-conjugating enzyme activity.
