SBP levels in 446 normal subjects and 46 patients with liver cirrhosis (LC) were evaluated by the radioimmunoassay procedure developed in this laboratory. In 201 normal males and 245 normal females, significant age-related declines before twenty years of age (p<0.001) and significant age-related increases at twenty years of age or over (p<0.001) were demonstrated in either sex. SBP levels in normal females were two-fold higher than in normal males at twenty years of age or over. SBP levels and sex steroids in 35 male patients with LC and SBP levels in 11 female patients with LC were compared with age- and sex-matched controls. SBP levels (μg/ml) in male and female patients with LC were 5.04±0.21 (mean±SE) and 5.93±0.65,which were both significantly higher than in the controls (p<0.001,p<0.01) respectively. SBP levels in 23 patients with compensated LC were significantly higher than those in 12 decompensated patients (p<0.05). Significant positive correlation was demonstrated between SBP levels and cholinesterase in 35 male patients with LC (r=0.35,p<0.05). Testosterone (T) levels in patients with LC were 5.16±0.37 (ng/ml), which was slightly lower than in the controls but insignificant. T levels in 8 patients with decompensated LC were significantly lower than in the controls and in 11 compensated patients (p<0.05 and p<0.05), respectively. Estradiol (E_2) levels in patients with LC were 40.9±1.9 (pg/ml), which was significantly higher than in the controls (p<0.01). E_2 levels in 8 decompensated patients were significantly higher than in 11 compensated patients. Significant positive correlation between free T and T/SBP was demonstrated both in the controls and patients with LC (r=0.90,p<0.001 and r=0.87,p<0.001), respectively. T/SBP levels in patients with LC were significantly lower than in the controls (p<0.01), being lower in 8 decompensated patients than in 11 compensated patients. Free T concentrations in patients with LC were 0.12±0.01 (ng/ml), which was significantly lower than in those of the controls (p<0.001). Free T concentrations in 8 decompensated patients were insignificantly lower than in 11 compensated patients. Free E_2 concentrations in the same patients were 1.22±0.11 (pg/ml), which was significantly higher than in the controls (p<0.01). Free E_2 concentrations in 8 decompensated patients were significantly higher than in 11 compensated patients (p<0.05). It was suggested from these results that the feminization in male patients with LC was due to the estrogen predominance associated with an elevation of SBP levels.
