A case of 45-year-old Japanese woman with a presurgical diagnosis of uterine leiomyoma underwent a total abdominal hysterectomy and salpingooophorectomy. Pathologic examination revealed an endometrial stromal nodule. Although there is no reliable preoperative diagnostic procedure for identifying this entity, it should be included in the differential diagnosis of leiomyoma-like uterine lesions.
