Purpose : To determine the maximum tolerated area under the concentration versus time curve (MT AUC) and to estimate the optimal AUC of carboplatin (CBDCA) administered daily combined with concurrent thoracic radiotherapy (TRT) in elderly patients with non-small cell lung cancer (NSCLC). Patients and Methods : Twenty-three patients were enrolled and 22 were assessable. The median age was 75 years (70 to 85 years). The clinical stage was I in be consistent in 4 patients, IIIA in 9 patients, IIIB in 8 patients and IV in 1 patient. The total dose of CBDCA was calculated by the Calvert's formula and divided into 25 single doses administered 5 times per week. TRT was adoministred at a total dose of 50 to 60 Gy by a single daily dose of 2 Gy within 1 hour after CBDCA administration. The starting AUC was 4 mg・min/ml and was increased at increments of 2 mg・min/ml when the safety was confirmed. Results : The MT AUC was 12 mg・min/ml. The recommended AUC was 10 mg・min/ml (0.4mg・min/ml/day). The actual measured AUC at each dose level could be basically adjusted to the target AUC, and the AUC of CBDCA showed a significant correlation with the percent decrease in neutrophil counts (r=0.71,p<0.001), and platelet counts (r=0.61,p<0.003). The overall response rate was 68%. The median survival was 13 months, and 1-year survival was 59% in stage III patients. Conclusion : The combined modality treatment of daily CBDCA administered using the AUC target formula and TRT made possible the prediction of toxicity, and was effective in elderly patients with NSCLC.
