The basic study on skeletal muscle fiber regeneration after various muscle injuries has become increasingly important in the understanding of the clinical courses and prognosis of patients with muscle injuries. The author studied difference in appearance of the muscle fiber regeneration after ischemia from that after traumatization in the tibial anterior muscles (TAM) of rats, besides histologically HE and Gomori-trichrome staining, adopting an immunohistochemical technique with anti-laminin antibody and histochemical methods for SDH, ATPase and acid phosphatase. In 40 rats, the abdominal aortas were ligated, and the ischemic injury of TAM was observed in thirteen rats. The blood flow measured with laser-Doppler spectroscope fell to 0 just after the ligation, but it returned to a degree of the normal range in 38 of the 40 rats. On the other hand, the midportion of TAM was crushed with a clamp in six rats for traumatization. For each of these rats the TAM was light microscopically investigated on the 7th day after the operation according to the techniques described above. The simple ligation of the abdominal aorta caused a severe ischemic injury only in three of the 40 rats examined while the crushing caused muscular destruction without fail. The results of the blood flow and the histological study seem to indicate that the collateral circulation started within less than two hours after the ligation in most of the rats. Phagocytosis of the necrotic muscle fibers was completed faster in the crushed TAM than in ischemic TAM. But granulation tissue was formed in the former, even inside the original basal lamina tubes of necrotic fibers. And newly formed laminin appeared to encircle the myotubes inside the original basal lamina tubes in both kinds of the lesions. In ischemic lesions, laminin of the original basal lamina disappeared after the myotubes matured, although it remained in the crushed lesions. Fibrous connective tissue from the granulation tissue might influence the disappearance of laminin of the original basal lamina. Thus, it is concluded that the collateral circulation is not functioning under a normal situation and assumed to start functioning within less than two hours after the ligation, and that the outcome of muscle regeneration varies according to the types of muscle injuries depending on the changes of basal lamina structure. In clinical practice, this variation has to be taken into consideration in the treatment and in the prediction of recovery of patients with muscle injuries.
