This is a report of 2 cases of posterior shoulder instability due to batting in baseball players. [Case 1] A 15-year-old boy felt a pain in his left shoulder when he strongly swung in a ball game. After an injury, he frequently felt shoulder pain during playing baseball. At the time of testing, he had posterior instability and severe pain in the left shoulder. MR images showed detachment of the posterior labrum. 4 months after the 1st episode, an operation was performed. Posterior Bankart lesion was observed and repaired arthroscopically using bioabsorbable anchors. [Case 2] A 17-year-old boy felt left shoulder pain when he strongly swung in a game. He occasionally felt pain and discomfort in his left shoulder at batting. He had click and instability of his left shoulder with posterior load and shift test. MR images showed detachment of the posterior labrum. 3 months after an injury, an arthroscopic posterior Bankart repair was performed. In both cases, the ROM exercises started within a week after the operation. After 16 weeks, they could be back in the ground. After a year, they did not feel pain or discomfort in the shoulder. The JSS shoulder instability score improved from 53 points to 100 points in Case 1, and from 58 points to 98 points in Case 2. In conclusion, we experienced a rare 2 cases of posterior shoulder instability due to batting. An arthroscopic posterior Bankart repair was very useful.
