Ovarian cancer is the leading cause of cancer death with women with a gynecologic malignancy. More than 70% of women with ovarian cancer present with advanced disease, most with extensive pelvic and upper abdominal metastasis. An important prognostic factor for both progression free survival and overall survival is the volume of residual tumor at the completion of "debulking surgery" It remains to be determined if the prognostic factor is residual disease or efficacy of the debulking surgery. There is a large range of success in debulking surgery depending on treatment venue , and data suggests that women treated in aggressive centers have improved survival. Opportunities for effective surgery range from identifying women who will benefit from risk reducing surgery to radical debulking operations including resection of the rectosigmoid colon, and even secondary salvage surgery. This talk will focus on the scientific basis of debulking surgery, the anatomy that facilitates optimal surgery, techniques for accomplishing this procedure, and the selection of patients for salvage surgery.
