脳卒中後疼痛に対する脊髄電気刺激療法の有効性

Post-stroke pain is most difficult to treat, and motor cortex stimulation (MCS) is effective on approximately 50% of the patients. However, MCS is very expensive and invasive treatment. On the other hand, spinal cord stimulation (SCS) is less invasive and reported to be effective on some patients with post-stroke pain. However, there has been no report about the analysis of large number of SCS of post-stroke pain. We performed SCS in 16 patients (9 male, 7 female, 37 - 72 y.o.) with post-stroke pain. In 16 patients, seven suffered from thalamic lesion, seven putaminal hemorrhage, one cortical infarction and one brainstem infarction. Two patients complained of hand pain and the others foot pain mainly. Seven (44%) patients showed good pain relief, and two showed fair. The others showed poor response. Finally seven patients received stimulation device. From our results, SCS should be tried for post-stroke pain, because of its less invasiveness and effectiveness.
