allopurinolの血管リモデリング抑制効果とその機序の検討

高尿酸血症は心血管性疾患の危険因子のひとつであり, その治療は近年重要視されてきている. しかし疫学的な調査に対して, 高尿酸血症が動脈硬化進展をもたらすメカニズムはほとんど検討されていないのが現状である. 我々は,SHRの総頚動脈結紮モデルを用いxanthine oxidase(XO)阻害薬;allopurinolが血圧・全身状態を変化させること無く, 血管の内向きリモデリングを抑制しうることを示した. さらに培養平滑筋細胞においても尿酸は増殖刺激作用を有することが確認されたため, このリモデリング抑制作用はallopurinol投与により平滑筋増殖が抑制されたためと考えられた. 高血圧を呈する病態において XO/xanthine dehydrogenase (XD) 反応・尿酸が血管のリモデリングひいては動脈硬化進展に関与している可能性が示唆された.
