Two Cases of Cutaneous Mycobacterium chelonae Infection

Two cases of M. chelonae infection in immunocompromised hosts. Case 1: A 24-year-old man presented with several lesions of subcutaneous abscesses on his right arm. He had been treated for 5 years with prednisone and azathioprine for dermatomyositis. M. chelonae infection was diagnosed by culturing pus discharge from the abscess. After susceptibility testing of the organism, oral clarithromycin, 200mg twice daily, was started. The skin lesions improved after 5 months of treatment. Case 2: A 58-year old man presented with a subcutaneous nodule on his right foot that had appeared 6 months after trauma. He had been treated for 5 years with prednisolone and cyclophosphamide for pemphigus vulgaris. M. chelonae was identified by culture of a skin biopsy. Three weeks after starting oral clarithromycin, 400mg twice daily, the skin lesions showed improvement and the patient continued clarithromycin for the following 6 months. However, new lesions appeared 27 weeks after starting the antibiotic. We further added oral prulifloxacin, 200mg twice daily. Therefore, lesions have gradually improved, and no aggravation has been observed in the following 14 months. In both cases, the patients were immunocompromised due to receiving immunosuppressant drugs, and lesions were restricted to the skin. In case 2, skin lesions continued after taking oral antibiotics for 21 months. This may indicate that there is a limit to the treatment with antibiotics for M. chelonae infection. Finally, we analyzed treatments for M. chelonae infection in previous cases that have been reported in Japan, and referred to the utility of drug susceptibility testing.Skin Research, 11: 96-102, 2012
