Complicated Case of Anti-Laminin Gamma-1 Pemphigoid in Psoriasis

A 62-year-old man was diagnosed with psoriasis vulgaris around the age of 42 years. The psoriasis exacerbated, and the patient developed bullae in August 2010. He was referred to our hospital on November 2, 2010. At the initial visit, scaly edematous erythema was observed over the whole body and tense bullae were scattered. Anti-BP 180 antibodies were not detected. Subepidermal bullae were found on histopathological examination, and infiltrating cells were composed of mainly neutrophils and a small number of lymphocytes. Direct immunofluorescence showed deposition of immunoglobulin G (IgG) and C3 in the basement membrane zone. Immunoblotting showed IgG reactivity to recombinant protein for the C-terminal fragment of human laminin gamma-1. The patient was diagnosed with anti-laminin gamma-1 pemphigoid associated with psoriasis vulgaris on the basis of these findings. The bullae resolved with administration of 60mg/day prednisolone, and no exacerbation has occurred since then.Skin Research, 11: 75-81, 2012
