腰部脊柱管狭窄症の保存的治療について神経ブロック的手技を中心に紹介する．腰部脊柱管狭窄症はさまざまな症候を呈する一症候群ととらえるべき病態の可能性がある．そのため，症状やさまざまな画像所見などをもとに症例ごとに狭窄の部位，硬膜外腔の癒着の有無など責任病巣の所在を的確かつ迅速に診断し，治療方針を決定していくことが重要と思われる．腰部脊柱管狭窄症に対する神経ブロック的手技では，硬膜外ブロックや神経根ブロックなどに加え，椎間板造影や腰部交感神経節ブロックも治療の一手段として有効であると考えられる．
