麻酔導入後に原因不明の開口度の低下をきたした１例

麻酔導入直後に開口度が低下し挿管困難となった症例を報告する．患者は64歳，男性．下顎隆起が大きかったため，Mallampatiのclass分類IIIであったが，意識下で十分な開口量を確認したので急速導入を行った．導入直後に急激に開口度が低下し挿管困難となった．数回の強制開口によりエアウェイスコープが挿入可能となり，経口挿管に成功した．術後には開口度の低下はなく，顎関節に疼痛などもなかった．この原因として，関節円板の復位を伴わない異常によるクローズドロックの可能性があると考えられた．
