A Case of a Type 1 Diabetes Child with a Type 1 Diabetic Mother.

We observed the manifestation of type 1 diabetes mellitus in a 2-year-old girl (Case 2), whose mother was a 42-year-old type 1 diabetic (Case 1) patient. The mother had been diagnosed as having type 1 diabetes mellitus at 18 years of age because of polyuria and polydipsia. Nine months after the diagnosis, insulin therapy was started. At age 39, she became pregnant and her hemoglobin A1c (HbA 1c) level was 9.8% at that time. As she wished to continue the pregnancy, she was hospitalized for strict glycemic control. After 10 weeks of pregnancy, her glycemic control was stable based on an HbA1c level of approximately 5%. At 38 weeks and 2 days of pregnancy, her child was delivered by Caesarian section. Case 2, presently a 2-year-old girl, weighed 2865 g at birth which was appropriate for date (AFD), without neonatal complications. She gained weight adequately with mixed feeding, and her growth and development were normal with no tendency for obesity. She manifested polyuria and polydipsia at the age of two, and her blood glucose level was 510 mg/dl. She was admitted to the pediatric ward of our hospital, and insulin therapy was initiated. Her HbA1c level was 8.8% at that time and subnormal endogenous insulin secretion was observed. We have experience of many pregnant diabetic women in our hospital, and have followed their progress. This is the first Japanese pediatric case in our hospital, born to a diabetic mother, manifesting type 1 diabetes mellitus.
