Effects of Imidapril Treatment on Insulin Resistance in Diabetic Patients

Japanese Guidelines for the Management of Hypertension (JSH 2000) published in June 2000 recommended that hypertensive diabetics lower their blood pressure to less than 130/85mmHg. Angiotensin converting enzyme (ACE) inhibitors are regarded as a choice for first-line drugs. Some ACE inhibitors improve insulin resistance, but little evidence is available on the effects of imidapril. o determine the effects of imidapril on insulin sensitivity, we studied 10 patients with hypertension and diabetes mellitus. Euglycemic clamp at insulin infusion of 40 mU/m<SUP>2</SUP>/min and 400 mU/m<SUP>2</SUP>/min was done before and after 12 weeks of treatment with 5mg of imidapril. Blood pressure fell from 154/85mmHg to 138/76 mmHg (p<0.05). In euglycemic clamp, the glucose infusion rate (GIR) increased from 4.6±2.6mg/kg/min to 5.5±2.1mg/kg/min (p<0.05) at 40 mU/m<SUP>2</SUP>/min of insulin, but no statistical difference was seen in GIR at 400mU/m<SUP>2</SUP>/min. These results suggest that the ACE inhibitor imidapril decreases blood pressure and improves insulin sensitivity.
