A Case of Diabetic Patient with Anterior Tibial Compartment Syndrome Triggered by Muscle Cramp.

We present a case of a diabetic male 53 years of age.After the onset of diabetes at 48 years, his glycemic control remained poor.After alcohol abuse.he developed severe leg cramp which was sustained overnight, and the cramp was complicated with swelling of the left anterior tibial area.After admission fever, GOT, LDH, and CPK were elevated.At the anterior tibial area, CT scan revealed low density area and echogram revealed swelling and high echo level.Doppler echogram revealed intact main arterial perfusion, low venous flow and increased by-pass flow.He gradually developed tibial nerve paralysis.Surgery was performed to reduce the intrafacial pressure.A pathological study of affected muscle revealed ischemic necrosis.<BR>He was diagnosed with acute anterior tibial compartment syndrome.The syndrome was thought to be induced by sustaining muscle cramp due to various factors such as long-standing hyperglycemia, liver dysfunction, peripheral neuropathy, and alcohol abuse.<BR>To our knowledge, there has been no previous report of anterior tibial compartment syndrome in a patient with diabetes mellitus. Therefore, this case suggests the importance of considering the anterior tibial compartment syndrome in diabetic patients with cramp and diabetic neuropathy, and provides some evidence of the pathophysiology of this condition.
