Glomerular Clearance of Various Urinary Proteins in Patients with Non-Insulin Dependent Diabetes Mellitus (NIDDM) Complicated by Microalbuminuria.

Glomerular clearance (GC) of urinary proteins with various molecular sizes and charges were determined in 14 patients with NIDDM complicated bymicroalbuminuria.Sixteen healthy volunteers were recruited as controls.The GC rates of urinary proteins were determined by obtaining glomerular urine by the L-arginine infusion method, which reversibly inhibits tubular reabsorption of urinary proteins.<BR>In healthy volunteers, GC values were in the following order;β<SUB>2</SUB> microglobulin>α<SUB>1</SUB> microglobulin>IgG κ-chain (dimer)>IgG-1>transferrin>IgG 4≥albumin.The GCs of urinary proteins with molecular weights less than 50kd (κ-chain dimer) decreased exponentially with larger molecular size.However, the GCs of urinary proteins withmolecular weights exceeding 50 kd were within a narrow range (2-4μ<I>l</I>/min), were more anionically charged, and these GCs weresmaller.In NIDDM, the GCs of strongly anionic albumin and IgG 4 were greatly elevated, while that of cationic IgG-1 decreased reciprocally.<BR>These results strongly suggest that impaired glomerular charge selectivity contributes, at least in part, to the enhanced GC of albumin in NIDDM with microalbuminuria.
