Determination of Thresholds and Suitable Periods for Performing the Screening Test for Glucose Intolerance in Pregnant Women Using Random Blood Glucose Values.

In a group of 1733 pregnant women attending a prenatal clinic at Asahi General Hospital over an 18-month period (Sept. 1, 1988-Feb. 28, 1990), random plasma glucose values were investigated during two periods, i. e., before 16 weeks and between 26 and 28 weeks, and 75 g oral glucose tolerance tests (75 g OGTT) were performed on patients with levels exceeding 95 mg/d<I>l</I>. A proper threshold and recommendable conditions for screening for glucose intolerance were obtained. During both periods, sensitivity increased as the cutoff point was lowered until a level of 100 mg/d<I>l</I> was reached, but sensitivity was unchanged at cutoff levels below 100 mg/d<I>l</I>. The specificity of the screening test using blood glucose values measured within 2 hours after breakfast, was less than half the specificity using values measured 2 hours or more after breakfast. The mean values and SD measured 2 hours or more after meals in normal pregnant women were 83.0±8.4 mg/d<I>l</I> before 16 weeks and 81.0±8.0 mg/d<I>l</I> between 26 and 28 weeks. The variation among values measured after 2 hours or more is small and stable, and 90% of each value was below 100 mg/d<I>l</I>. Finally, 5.6% of all pregnant women were found to have glucose intolerance, and it was detected in 60% of them before 16 weeks. It is recommended that screening tests using random blood glucose values should be performed using 100 mg/d<I>l</I> as a threshold, during both the period before 16 weeks and between 26 and 28 weeks, and that blood glucose values be measured 2 hours or more after meals.
