A 77-year-old man was admitted in September1990because of frequent episodes of fasting hypoglycemia and postprandial hyperglycemia. He had been treated with insulin since1979. Fasting plasma total IRI was 1770 μU/m<I>l</I>, and free IRI was 124 μU/m<I>l</I>. Scatchard analysis of insulin antibody levels demonstrated a high capacity (41.6nM) of high-affinity binding sites. In a euglycemic clamp study, insulin infusion at the rate of 10 mU/kg/min induced a marked increase in total IRI to 5550 μU/m<I>l</I>, whereas free IRI showed an only minimal increase to 67 μU/m<I>l</I>, and the metabolic clearance rate of glucose was essentially unchanged. After insulin treatment was stopped, the capacity of high-affinity binding sites gradually decreased, and fair glycemic control without hypoglycemia was attained. We conclude that the patient's fasting hypoglycemia and postprandial hyperglycemia were mainly due to an alteration in insulin pharmacokinetics induced by insulin-binding antibodies.
