Cases of Giant Pyogenic Liver Abscess Accompanying NIDDM.

Pyogenic liver abscess, as a serious complication of diabetes, has been reported only rarely in Japan, even though diabetic patients have impaired host defenses. We have encountered three cases of pyogenic liver abscess accompanied by diabetes mellitus. We analyzed the clinical features and pathological characteristics of these abscesses in comparison with other cases reported to date in Japan.<BR>Our first case was a 69-year-old man with a fever and jaundice. The second case was a 61-year-old man admitted because of fever and right hypochondralgia. The third case was a 74-year-old man whose right visual acuity had diminished markedly and who had a fever and epigastralgia. All three cases had a solitary liver abscess. Klebsiella pneumoniae was detected as the pathogenic bacteria in two cases: no pathogenic bacteria were detected in the other case. Each case was completely cured by treatment with insulin, antibiotics and drainage of the abscess.<BR>Diabetes should be recognized as a predisposing cause of pyogenic liver abscess. If a diabetic patient develops a fever, it is important to investigate the abdomen by abdominal echography and CT.
