Pyogenic vertebral osteomyelitis, a serious compliacation of diabetes, is extremely rare in Japan. However, as knowledge about pyogenic vertebral osteomyelitis has accumulated, case reports on this combination have been gradually increasing recently.<BR>A i6-year-old man with NIDDM poorly controlled by insulin, was admitted to our hospital with sustained fever and lumbago which had not responded to antibiotic therapy. Bone and gallium scan studies revealed an abnormal accumulation of isotopes in the bodies of L2 and L3. Magnetic Resonance Imaging (MRI) revealed inflammatory changes in the body of L2 and in the surrounding tissues. The clinical diagnosis of pyogenic vertebral osteomyelitis was confirmed by bone biopsy.<BR>When a diabetic patients suffers from sustained fever and lumbago suggesting pyogenic vertebral osteomyelitis, bone scan and MRI are of clinical value in the early detection of the bone lesions. MRI is of great value in differentiating the purulent inflammatory changes from other disorders.
