Effects of Recombinant Human Erythropoietin on Diabetic Nephropathy with Anemia.

Anemia is one of the complications which occurs in patients with chronic renal failure due to diabetic nephropathy. Recently recombinant humanerythropoietin (EPO) has been found to be effective in treating chronic renal anemia, due to a variety of causes during hemodialysis. However, there have been few reports investiging the effects of EPO on the anemia caused by diabetic nephropathy in the pre-dialysis stage.<BR>Five anemic NIDDM patients with diabetic nephropathy [mean ages: 61±6yr., duration of DM: 18±3yr., erythrocyte counts: 245±23×104/mm3, hemoglobin concentrations (Hb): 7.9±0.8g/dl, serum creatinine (s-Cr): 4.3±1.4 mg/dl, BUN: 56±9 mg/dl] were treated with EPO and followed for 28 weeks. EPO was administered intravenously once a week, starting with a dose of 3000, which was then increased to 6000, and finally 9000 units.<BR>After EPO treatment; 1) The anemia was significantly improved. At 12 weeks, the erythrocyte counts were increased to 325±17<SUP>+</SUP>×104/mm3, and Hb to 10.1±0.6<SUP>+</SUP>g/dl, 2) By the linear regression analysis of the reciprocal values for s-Cr., the rates of reduction in the residual renal functions were significantly decreased from-0.038±0.009 to-0.0054±0.016<SUP>+</SUP>dl/mg·month, 3) There was a significant reduction of 24 hour urinary protein excretion after 28 weeks of EPO treatments (49.7± 14.3<SUP>+</SUP>%, +: p<0.01).<BR>From this study, EPO was found to be effective in treating anemia caused by diabetic nephropathy even in the pre-dialysis stage. There were no adverse effects on renal function resulting from EPO administration. However, further studies will be necessary to elucidate the precise mechanism of EPO therapy on renal function.
