Characteristics of Diabetic Patients with Basedow's disease.

Eleven patients with diabetes mellitus (Male/Female: 2/9, IDDM/NIDDM: 5/6) complicated by Basedow's disease were found among 440 patients with DM (IDDM/NIDDM: 46/394) and 270 patients with Basedow's disease.<BR>Statistical analysis showed the mean HbA<SUB>1</SUB>c of 5 IDDM patients with Basedow's disease to be significantly lower than in the control subjects.With regard to diabetic complications, while 5 IDDM patients with Basedow's disease had no proteinuria, no significant differences were observed in the incidence of retinopathy or neuropathy.<BR>A family history of DM was present in 5 of the 11 cases and of thyroid disease in 3.Analysis of the HLA antigens of these 11 patients revealed HLA DR-4 in all 5 IDDM patients with Basedow's disease and in 4 of the 6 NIDDM patients with Basedow's disease.<BR>These findings indicate that both genetic factors and autoimmune mechanisms play importantroles in relation to complication of IDDM by Basedow's disease, and while genetic factors arebelieved to play a role, the role of autoimmune mechanisms in complication of NIDDM by Basedow'sdisease is undeniable.
