Acute hepatic injury during diabetic ketoacidosis. A case report.

The elevation of serum transaminases during diabetic ketoacidosis (DKA) is rare and its etiology is not clear. We encountered a case who developed elevation of serum transaminases during DKA. Since this 36-year-old female with insulin dependent diabetes mellitus had discontinued insulin administration by herself, she was admitted to a community hospital for treatment of DKA. Because of the lack of basic insulin administration, acidosis continued over 3 weeks, followed by the appearance of hepatomegaly and rapid elevation of sGPT. By means of continuous intravenous insulin infusion, DKA and the elevated sGPT levels were quickly improved. Hepatitis related virus markers were negative and the sGPT level remained normal for over one year. Histological study of the liver biopsy revealed only slight lymphocytic infiltration and moderate fatty metamorphosis of hepatocytes. Infiltration of the CD8 T cell was not found. As causes of the elevation in sGPT, aberrant glucose metabolism and persistant DKA are considered.
