A case of pyogenic vertebral osteomyelitis associated with diabetes mellitus is reported. A 72-year-old male who had had diabetes for 20 years was admitted to our hospital because of fever and lower back pain. Physical examination revealed a knock pain in the region from L 2 to S 2. He was restricted to bed because of severe pain radiating from the lower back to both thighs. Laboratory data showed leucocytosis (19, 500/mm<SUP>3</SUP>), elevated ESR (78mm/h), positive CRP (6+) and pyuria. Alpha-streptococci were idnentified in a culture of arterial blood. Tuberculin skin test was negative. Plain X-ray and CT examinations revealed osteolytic lesions at the L 3 and L 4 bodies. Bone scintigram using <SUP>99m</SUP> Tc-MDP showed a marked accumulation of radioactive grains at L 3 and L 4. From these findings, a tentative diagnosis of pyogenic vertebral osteomyelitis was made, and antibiotic chemotherapy against alpha-streptococci was started. After 3 months of chemotherapy, inflammatory signs disappeared, and he was completely relieved from lower back pain and gait disturbance.<BR>Although pyogenic osteomyelitis is a rate disease, it is reported that 25-35% of cases are associated with diabetes mellitus, especially those with urinary tract infection. Since chemotherapy is effective at on early stage, this disease should be borne in mind when a patient complains of fever and back pain.
