To assess the neccesity of insulin treatment for diabetics, we evaluated multiple clinical manifestations of diabetics with special reference to the capacity of insulin secretion on a glucagon loading test (GT) by discrminant function analysis. We performed GT on 20 diabetics treated with insulin injection (group I) and 51 diabetics controlled by diet or an oral hypoglycemic agent (group N). The clinical manifestations of age at onset of diabetes mellitus (DM), duration of DM, obesity index, hemoglobin A<SUB>1</SUB> (HbA<SUB>1</SUB>), fasting blood plasma glucose (FPG) on GT and peak level of serum Cpeptide reactivity (CPR) on GT were used as disciminatory variables. The discriminant function effective for discrimination between group I and group N was calculated with these six variables. The results were as follows. Peak level of serum CPR on GT (X<SUB>1</SUB>), age at onset (X<SUB>2</SUB>) and FPG on GT (X<SUB>3</SUB>) were selected as effective variables for discrimination, and the linear discriminant function with these three variables was expressed as Z=2.174 X<SUB>1</SUB>+0.071 X<SUB>2</SUB>-0.020 X<SUB>3</SUB>-4.060. The rate of mis-classification by this function was 0.050 in group I, 0.137 in group N and 0.113 in all 71 diabetics. It is suggested the linear discriminant function with peak level of serum CPR on GT, age at onset of DM and FPG on GT is effective in assessing the necessity of insulin treatment for diabetics.
