Clinical study of a case of prolactinoma associated with diabetes mellitus.

A case of prolactinoma in a 36-year-old male diabetec is presented. His diabetes mellitus was detected at 33 years of age because of hyperglycemia, ketonuria, thirst, and polyuria. Since then he has been treated with an oral hypoglycemic agent followed by insulin therapy. He was admitted to our clinic because the hyperglycemia was resistant to insulin therapy and symptoms suggesting a pituitary tumor gradually appeared. Endocrinological studies revealed that the pituitary tumor was a prolactinoma. Marked improvement in laboratory findings including serum triglyceride (TG), fasting blood glucose (FBG), serum prolactine level (PRL), and CEA, and of neurological symptoms of the pituitary tumor was obtained by Bromocriptinc (7.5mg/day) and intravenous insulin infusion. He underwent a trans-sphenoidal hypopliysial adenomcctomy after Bromocriptine therapy for two weeks and the abnormally elevated laboratory findings before surgery had normalized after one month. The hyperprolactinemia and/or hypothalamic derangement due to the pituitary tumor might have been responsible for the metabolic disorders seen in this patient.
