In 1924, Meleney reported hemolytic streptococcal gangrene caused by group A β-hemolytic streptococci. In 1948, the origin of this infectious skin disease with necrotic change was located in superficial fascia, and Wilson named this skin diseaes necrotizing fasciitis in 1952.<BR>We report one case of necrotizing fasciitis associated with non-insulin-dependent diabetes mellitus. A 77-year-old man, who had a 20-year history of diabetes mellitus, was transferred to Tokyo Women's Medical College Hospital, suffering from fever and a cellulitis-like skin lesion of the perianal area. In spite of the administration of antibiotics and insulin, the area of redness and swelling spread, and a diagnosis of necrotizing fasciitis was made. Although extensive surgical debridement was carried out, 9 days after he was transferred pulmonary candidiasis and meningitis occurred, and on the 31st day he died of respiratory and renal failure. Necrotizing fasciitis is an uncommon disease and is knwon for its high mortality rate, especialy in diabetics. Therefore, it must be differentiated promptly from purulent cellulitis and diabetic necrosis, as it is essential to carry out surgical debridement immediately in order to improve the life expectancy.
