Two cases of Kleinefelter's syndrome associated with diabetes mellitus, and analysing the clinical features of all domestic reported cases.

We have reported two cases of Kleinefelter's syndrome with diabetes mellitus and have analyzed all the cases from the referenes reported in Japan to the end of March 1984. The first case, aged 57 and exhibiting 46 XY XXY, mosaichism in chromosome examination, was complicated with non insulin dependent diabetes mellitus, IIb type hyperlipidemia and ischemic heart disease. The second case, aged 23 with 47 XXY, pure type, was found to have diabetic ketoacidosis (insulin-dependent diabetes mellitus) complicated by hypertrophic cardiomyopathy and psycopathic disease, i.e. idiocy. The diabetic control was good in the first case with diet therapy only, but not in the second without insulin injection. Good control in these two cases was obtained after discharge from the hospital.<BR>We have concluded from the analysis of the clinical features in 17 reported cases that: 1) Seven out of 11 reported cases (64%) were insulin-dependent diabetes (type I diabetes); 2) in the glucose tolerance test, insulin secretability seldom exists and hyperglycemia (diabetic pattrn) was observed in 10 out of 15 cases (66%); 3) nine out of 11 cases were not well controlled without medication with diabetic therapeutics; and 4) three out of 11 cases (27%) had diabetic familial histories in close relatives.<BR>The endocrinological findings in the cases reported in Japan are not much different from those reported abroad.<BR>The frequency of complicated diabetes mellitus in Kleinefelter's syndrome in Japan is very low (3.9%) compared with that elsewhere. We conclude from this that complicated diabetes mellitus in Kleinefelter's syndrome seems to be uncommon in Japan even now, but the frequency percentages should rise as the characteristics of complicated diabetes mellitus in the syndrome come to be widely elucidated.
