With the aim of studying the abnormality of the diabetic placenta both histopathologically and quantitatively, we examined the structure of the fetal capillary of the terminal villus in 19cases of diabetic placenta using the digigrammer system.<BR>In the diabetic placenta, the ratio of the area of the capillary of the terminal villus to the total area of the terminal villus assumed to be related to the fetal circulation flow volume, was 29.19±6.13%. This was a significant decrease compared with the 50.23±5.61% in the normal placenta (p<0.001).<BR>Moreover, the same phenomenon can be seen in the ratio of the vascular syncytial membrane (VSM) to the circumference of the terminal villus, which seems to have affected the exchange of materials between mother and fetus, especially the oxygen supply which was 12.79±4.86% in the diabetic placenta and 37.12±8.12% in the normal placenta (p<0.001).<BR>Typically, a diabetic placenta causing fetal death showed markedly low ratios: 30.94% for the ratio of the area of the capillary of the terminal villus to that of the terminal villus, and 9.87% for the ratio of the VSM to the circumference of the terminal villus.<BR>In conclusion, it can be said that the diabetic placenta results from a decrease in the flow volume in the fetal capillary of the terminal villus and, along with it, from the structural point of view, exchange of materials between mother and fetus, particularly the oxygen supply, would be interrupted. What has been proved is that the diabetic placenta can easily cause a fetus to have a hypoxic attack.
