High-density lipoprotein chlesterol (HDL-Chol) levels in Japanese diabetic patients were studied, with special reference to the protective role against the development of coronary heart disease (CHD). The subjects studied were 570 diabetic patients (346 males and 224 females), who were followed up until the end of 1982 with an average observation period of 7.5 years. HDL-Chol was determined by the dextran-magnesium precipitation method.<BR>1) Mean HDL-Chol levels in diabetic patients were higher in females than in males and also higher in non-obese than in obese subjects. Among non-obese subjects, insulin-treated patients showed significantly higher levels of HDL-Chol thandid. patients undergoing other types of treatment. No apparent relationship was obtained between the HDL-Chol level and the fasting glucose value determined concurrently.<BR>2) The incidence of CHD in diabetic patients treated with diet or hypoglycemic agents was significantly elevated in the male subjects with an HDL-Chol level of less than 35 mg/d<I>l</I> and in the female subjects with that of less than 45 mg/d<I>l</I> and was low in the subjects with an HDL-Chol level greater than 55 mg/d<I>l</I> in both sexes. The risk analysis and multiple logistic analysis indicated that the incidence of CHD was significantly related to age and ischemic ECG changes in addition to a decrease in HDL-Chol levels and an increase in the atherogenic index (LDL-Chol/HDL-Chol).<BR>In conclusion, it was evident that HDL-Chol was a protective factor against the development of CHD in Japanese diabetic patients as observed in the West.
