The case of a 66-yr-old slightly obese male patient with hyperosmolar hyperglycemic nonketotic coma (HHNC) whose serum antibody titers to influenza A-H<SUB>3</SUB> (Hong Kong flu) remained at extremely high levels (x512-x2048) for 18 months or more after recovery from coma, is described. Serum islet-cell surface antibody (ICsA) was also demonstrated at the initial stage of recovery. Determination of the HLA typing revealed A<SUB>2</SUB>, BW<SUB>4</SUB>, B<SUB>7</SUB> types.<BR>Although it was not possible to obtain direct evidence that influenza A-H<SUB>3</SUB> virus had caused insulitis leading to subsequent diabetes in this patient, it is interesting to have found ICsA in his serum soon after the onset, since ICsA is accepted as a highly organspecific, immunoresponsive humoral antibody. The fact that ICsA became negative with diabetic control, also supports a viral etiology in the development of the diabetes. On this basis, the authors are inclined to believe that the pancreatic islets in this patient may have been damaged following the virus infection to an extent capable of recovering normal function.
