The purpose of the present study is to clarify the possible role of abnormal vitamin D metabolism in the pathogenesis of osteopenia in diabetics.<BR>Seventy-five females over the age of 50 years were studied.These subjects were divided into two groups; control group (n=23) and diabetic group (n=52).<BR>Radial mineral contents (RMC) measured by photon absorptiometry in the diabetic group (0.44±0.01g/cm<SUP>2</SUP>) were significantly lower than those of control group (0.48±0.01g/cm<SUP>2</SUP>).Serum levels of 1, 25 (OH) <SUB>2</SUB>D in the diabetic group (35.2±2.7pg/ml) were also significantly lower than those of control group (47.3±4.5pg/ml).The intestinal absorption of <SUP>47</SUP>Ca, determined by the method of Avioli, in the diabetic group was significantly lower than that of the controls. Since the decreased <SUP>47</SUP>Ca absorption in the diabetics was in good agreement with the decreased serum levels of 1, 25 (OH) <SUB>2</SUB>D in the diabetics, it is possible to conclude that the decreased serum levels of 1, 25 (OH) <SUB>2</SUB>D have a causal role in osteopenia (i.e.) and decreased RMC, in elderly female diabetics.<BR>Further analysis in the diabetic group revealed that the duration of diabetes (r=-0.344, p<0.02), body weight (r=0.367, p<0.01) and age (r=-0.497, p<0.01) were significantly correlated with RMC and that the insulinogenic index (30 min.)(r=0.273, p<0.05) and age (r=0.277, p<0.05) were significantly correlated with serum levels of 1, 25 (OH) <SUB>2</SUB>D. RMC and serum 1, 25 (OH) <SUB>2</SUB>D levels of diabetics treated with oral hypoglycemic agents were lower than those of controls.<BR>These results might indicate that osteopenia is one of the complications of diabetes.
