Usefulness of vital staining with iodine solution in resection of early tongue carcinoma

The recurrence of oral squamous cell carcinoma (OSCC) is a major problem in the treatment of this disease. Often, epithelial dysplasia is found around the site of the original OSCC lesion. Sites where this has not been removed have been implicated in local recurrence and the development of secondary carcinomas. The purpose of this study was to determine the usefulness of iodine-staining for deciding the margin of excisional biopsy.<BR>One hundred two patients diagnosed with tongue squamous cell carcinoma at the Department of Oral and Maxillofacial Surgery, Tokyo Dental College, between 1987 and 2006 were selected. We classified the patients into two categories as follows: Group 1, where no iodinestaining was carried out, all treated between January 1987 and December 1996 (41 cases); and Group 2, where iodine-staining was carried out, all treated between January 1997 and December 2007 (61 cases). We compared clinical and pathological histories between these two groups.<BR>The results revealed that 83.6% of the patients in Group 2 were iodine-unstaining area. In Group 2 (8.2%) the lesion could be resected without secondary incision more than Group 1 (32.0%). Group 1 showed significantly higher local recurrence rates (17.1%) than Group 2 (4.9%). In the surgical margin, the epithelial dysplasia-positive rate in Group 1 was higher (26.8%) than that in Group 2 (8.2%). Staining with iodine solution was useful in defining the region of epithelial dysplasia. We believe that the iodine-staining method offers a useful tool in deciding the resection margin in OSCC.
