A case of double cancer of thyroid papillary carcinoma and squamous cell carcinoma of the lower gingiva is presented. Squamous cell carcinoma (T2N1M0) was diagnosed in a 64-year-old female patient, and tumor excision and radical neck dissection were performed. Histological examination revealed late metastasis from the thyroid papillary carcinoma, which was excised 10 years ago, to the cervical lymph nodes. In addition, it was suspected that thyroid carcinoma, metastasized to the lateral retropharyngeal node, because a low density mass in the retropharyngeal space was detected on CT at the initial examination. Thus a fine-needle aspiration biopsy under the guidance of CT was performed, showing no metastasis to the node. After the aspiration, CT examinations showed the mass had a tendency to disappear. Finally, at the second operation for reconstruction of the mandible, no mass was palpated. Hence the lesion detected at the initial CT examination was retrospectively diagnosed as lymphadenitis with cystic degeneration of the lateral retropharyngeal node
