The U.S.Center For Disease Control has reported that a dentist infected 5 patients with HIV.Since then, infection control during dental treatment has been attracting increasing attention.<BR>Internal contamination in an air turbine handpiece (called handpiece), particularly used to hollow out teeth, is suspected of being the source.<BR>In the present study, the processes involved in bacterial contamination of the handpiece was analyzed in a clinical study and two experimental studies. A device attached to the handpiece to prevent inter. nal contamination (abbreviated as AFCS) was compared with the use of dental vacuum suction and a conventional handpiece in terms of the number of comtaminating microorganisms.The oal bacteria were used as an indicator in these studies. The results were as follows:<BR>1.A conventional handpiece was heavily contaminated with bacteria when it was stopped while its tip was immersed in a bacterial suspension, and contamination spread up to the center of dental unit.However, when a handpiece equipped with AFCS was used, the center of the unit had no contamination.<BR>2.When the handpiece was stopped, the bur was kept in contact with the surface of the bacterial suspension.This result was that a conventional handpiece had no contamination in the center of the unit if vacuum suction was performed.<BR>3.When a conventional handpiece was clinically used with vacuum suction, the incidence of internal contamination decreased markedly. When a handpiece equipped with AFCS, no internal contamination occurred even if vacuum suction was not performed.
