Diagnostic Accuracy of Sialography for Salivary Gland Tumors.

Though sialography is useful in demonstrating the ductal system of the major salivary gland, its utility in the diagnostic evaluation of major salivary gland tumors remains uncertain.To determine the accuracy of sialography for major salivary gland tumors, we conducted a retrospective study on 70 patients and compared sialographical findings with respective histopathological findings. The sialography criteria used in the detection of masses had been as follows: 1) filling defect;2) displacement of the ductal system;3) blockage of the ductal system;and4) leakage of contrast medium. When criteria 3) and/or4) were found, malignant tumor had been diagnosed.<BR>The following results were obtained:<BR>1) Sialography accurately detected72%of parotid tumors and92%of submandibular tumors, and 100%of tumors greater than 4 cm in diameter, but only41%of tumors less than2cm.<BR>2) Sialography was useful in the diagnosis of only44%(4/9) of malignant parotid tumors and40%(4/10) of malignant submandibular gland tumors.<BR>3) On the basis of these results, it was suggested that the capacity of the sialogram to detect a mass less than2cm in diameter and to differentiate malignant from benign tumors was very poor.
