A Case of Ameloblastic Fibro-odontoma in the Mandible.

A 21-year-old female had swelling of the left cheek.<BR>Examination of the oral cavity revealed marked gingival swellingat the site of an unerupted left lower second premolar and first molar.<BR>Radiographic studies of the mandible revealed impacted teeth of he left lower second premolar and first molar and multicystic radiolucent lesion which contained numerous small radiopaque masses. Excisional biopsy revealed of ameloblastic fibroodontoma.<BR>Segmental mandibulectomy of the left mandible was done followed by mandibular reconstruction with multiphase biomedical implants consisting of hydroxyapatite, polytrifluoroethyl metacrylate, and titanium.One year and 10 months following reconstruction, the implant broke and had to be removed. A new mandibular reconstruction was accomplished using dacron-urethane mandible mesh tray and fresh autogenic particulate cancellous iliac bone, and marrow graft was performed.<BR>Orthopantomography performed two years and 6 months following the second surgery revealed mandibular continuity with normal bone.Reexamination at four years and 3 months following this surgery revealed no recurrence of the lesion.<BR>Histopathological examination of the biopsy specimen revealed a district border of the lesion.The tissue from the lesion contained proliferation of the juvenile connective tissue including tissue resembling the dental lamina and enamel organ as well as epithelial cell nests and calcified tissue resembling enamel and dentin.
