Oral complications and Oral Care in Bone Marrow Transplantation.

Today, bone marrow transplantation (BMT) is being used with increasing frequency, especially for the treatment of malignant hematological disorders, such as leukemia and severe aplastic anemia. It is considered that BMT should only be carried out through a team approach, covering expertise in a wide spectrum of disciplines such as internal medicine, dermatology, surgery, oto-rhino-laryngology, psychology, dentistry, etc. Complications following BMT often originate in the oral cavity and can cause morbidity and mortality in patients undergoing BMT, but in Japan the professional dentist is not yet a member of the team.<BR>In this paper, the clinical features and the pathogenesis of the oral sequelae of BMT are discussed. W e have proposed an oral care protocol in the BMT by dentists and emphasize the need for them to participate on the BMT team. The oral care protocol consists of 1) the removal of infectious focus from the oral cavity before BMT and 2) the surveillance and the topical treatment of various mucositis after BMT, such as chemo-radiative, bacterial, fungous, viral, GVHD mucositis, including diagnotic lip biopsies for GVHD cases.
