Three cases of reconstructed soft palate and oropharyngeal defects with free radial forearm flap are presented. Three male patients of forty-six (case 1) and fifty-one years (case 2, 3) of age had wide resection including radical neck dissection for advanced squamous cell carcinoma in the left retromolar trigone in case 1, in the right buccal mucosa in case 2 and in the oropharynx in case 3. All patients received surgical reconstruction using the forearm flap in the large defect of the oral mucosa and oropharynx. Postoperative speech function and velopharyngeal closure are satisfactory in all cases, using in combination with speech appliance. Consequently, the free radial forearm flap with speech appliance is considered to be suitable and functional for treating soft palatea nd oroph-aryngeal defects in which rehabilitation with conventional flaps or maxillofacial prosthesis is difficult.
