The Relationship between Prognosis and the Mode of Bone Resorptions by Carcinomas Involving the Mandible.

Fifty-six cases, diagnosed as squamous cell carcinoma of the mandibular gingiva, have been examined radiographically in an attempt to explain the relationship between modes of bone resorption and prognosis.<BR>We investigated radiographs on the marginal figures and the shapes of bone destruction, entrapments of bone and bone reactions.<BR>The results obtained were as follows<BR>1) Of the 56 patients, 47 (83.9%) showed radiographic evidence of bone involvement.<BR>2) According to the mode of bone resorption, the outlines of the radiolucent lesions were classified into well-defined type (40%), ill-defined tyye (24%) or irregular type (36%) and the shape of bone destruction was classified into well-defined type (44.7%), diffuse (permeative or infiltrative) type (44.7%) or moth-eaten type (10.6%).<BR>3) The entrapment of bone was detected in 28% of cases with bone involvement.<BR>4) The incidence of bone reaction was 11%.<BR>5) Well-defined outline or well-defined shape of bone destruction tended to have little recurrence and had good prognosis.<BR>6) Carcinomas that had ill-defined outline or moth-eaten appearance of bone destruction tended to recur often.<BR>7) Metastases after operation often occurred in patients with irregular type of outline or diffuse shape of bony destruction. Especially in irregular type, the incidence of metasasis was 66.6% and survival rate was 26.7%.<BR>These results suggest that the modes of bone resorptions reflect some of the characteristics of carcinoma and the present classification is helpful for planning the therapy.
