Clinical significance of hepaplastintest in hepatitis and liver cirrhosis

Hepaplastintest (HPT) was comparatively evaluated in 140 cases with liver diseases (9 fulminant hepatitis, 34 acute hepatitis, 50 chronic hepatitis and 47 liver cirrhosis), and in 31 normal subjects. The following results were obtained.1) The normal range of the activities of hepaplastintest was 80.3-134.3%.2) The activities of hepaplastintest showed a decrease in patients with hepatitis and liver cirrhosis as compared to normal subjects, especially in fulminant hepatitis and in liver cirrhosis with coma.3) In patients with hepatitis and liver cirrhosis, there was a correlation between the activity of hepaplastintest and the severity of the disease.4) An increase in PIVKA-inhibitor (more than 0.5), as an index of the complication of disseminated intravascular coagulation (DIC), was found in 1/3 of the the cases with fulminant hepatitis.Therefore, it is thought that the determination of hepaplastintest in hepatitis and liver cirrhosis is useful to determine the diagnosis and the prognosis of the diseases. Also involvement of DIC was suggested in fulminant hepatitis from the aspect of PIVKA-inhibitor.
