STENOSIS OF THE ANASTOMOSIS AFTER PANCREATOGASTROSTOMY TREATED WITH ENDOSCOPY

A 66-year-old man with intermittent upper abdominal pain was admitted for endoscopic treatment. He had a past history of pyrolus-preserving pancreatoduodenectomy reconstructed with pancreatogastrostomy due to cancer of the papilla of Vater three years before admission. The anastomosis of the pancreatic duct and the stomach had severe stenosis. Endoscopic incision with needle knife, balloon dilation, and stenting were performed. Two weeks later, acute pancreatitis due to stent occlusion occurred. The stent was removed, and acute pamcreatitis resolved soon. After that, no abdominal pain was recognized and dilatation of the main pancreatic duct was improved. One year later, the symptom recurred and the same endoscopic therapy was performed successfully to control the symptom. Efficacy of endoscopic treatment to relieve the stenosis of the anastomosis after pancreatogastrostomy was suggested.
