A 68-year-old female was visited to our hospital with the chief complaint of right lower quadrant pain. A hard mass was palpable in the ileocecal region. Barium enema study suggested the submucosal tumor with irregular shaped small depression. Colonoscopy showed hemispheric mass with central depression covered with normal mucosa, at the bottom of the cecum. Biopsy study revealed no malignancy. Histological study of the resected specimen revealed well differentiated tubular adenocarcinoma of the appendix with minimal cancerous invasion to the subserosa and massive inflammatory changes in the tumor.<BR> Preoperative diagnosis of primary appendiceal cancer is difficult. When submucosal tumor at the cecum with the nonvisualization of the appendix, appendiceal cancer should be suspected.
