Sarcoidosis is a granulomatous disease, occasionally involving the central nervous system (CNS), peripheral nervous system (PNS) and muscle. It has been generally believed that nervous system involvement of sarcoidosis is often refractory, because of the susceptibility of these nervous systems. In this paper we obtained information on sarcoidosis in neurology by means of a nationwide questionnaire survey. The questionnaire results demonstrated that sarcoidosis in CNS, PNS and muscle is rare. Therefore, sarcoidosis in such organs remains to be systematically investigated. The majority of sarcoidosis specialists consider sarcoidosis in the nervous system to be refractory based on a criteria where there is a combination of persistency, severity, and resistance against therapy. However, the definition of being refractory varied among doctors. It is important to work out guidelines for diagnosis and therapy of sarcoidosis in CNS, PNS and muscle.
