Relationship of Cutaneous Sarcoidosis to Pulmonary Involvement

This study was aimed to investigate the relationship between pulmonary and skin lesions in patients with sarcoidosis. Of 131 patients in which diagnosis of sarcoidosis was made by pulmonary biopsy, 45 patients (34%) had various cutaneous manifestations at onset of the disease.The mean age of onset of patients with skin lesions (52 years) was higher than that of patients without skin lesions (45 years).Between these two groups, no significant difference was found in frequency of epithelioid cell granulomas in transbronchial lung biopsy specimens, the rate of positive Gallium-67 uptake in lungs and the frequency of decreased pulmonary functions.In patients with skin lesions, 3 (7%) showed radiographic stage IV (irreversible fibrotic changes) disease, but it was 0% in non-skin sarcoidosis. Radiological clearing of the lungs within 3 years occurred in 50% of patients with skin lesions and 68% of patients without skin sarcoidosis. Skin lesions might predict a poor prognosis in sarcoidosis.
